EXACT same thing just happenned to me except I was on flat ground in 5th,and didnt hear or feal anything,just started to free-rev.
Clutch was installed for about 150 miles(previously broken in before the engine was replaced,ut still looked new)

Trying to decide wheterto get another disc(maybe sprung) and keep the pp or what

My pics are IDENTICAL to these...

We had a F350 with a 6speed swap that did that to 2 disk. On the third disk we noticed at the top of the bellhousing was some spots that were rubbing something. The block had a cam cover that was causing a very slight misalignment. We ground the edge off and been no problems since. Since you have done a swap make sure there is not something holding off the bellhousing.
