That idle screw being all the way in doesn't sound right. Have you gotten the vacuum leak fixed?

vacuum leak is fixed afaik. didn't hear it after a boost leak test and replacing the gasket. Only really heard it past 17psi anyways (before)

It might be time to get down to brass tacks and go through the FSM diagnostics step-by-step.

Do you know a small boy to help me test the AICV?

Does the creepy Nissan mechanic in the FSM call for a small boy in the diagnostics? I wouldn't put it past him...

have you checked your tps? Cut the wheel all the way one direction and hold it and turn the A/C on and see if the idle stays the same. If it does your iacv is functioning properly. My guess is that something is telling the ecu the wrong thing and it's telling the iacv to open a little too much.

I just read what you said about the clutch. If you put a load on the motor and it dies your iacv is not functioning properly...period.

If that's the case then I'll look for one at the junkyard next time I'm there.
