Well, change is done, and considering this is the 2nd disc that was shredded, the input shaft is starting to get worse for wear. I will need to replace that soon, probably in the next 4-6months. That, coupled with a brand new full clutch setup, should solve it all.
For those interested, this 2nd disc did the exact same thing. Inner splines completely stripped off. The flywheel was replaced with one that had less machining, and you can definitely tell the difference in step height between the two. Can't say for certain that was part of the issue, but I wouldn't disregard it either.
