Maybe the flange is warped causing you to stretch the bolts to much to get a good seal.

drill a tiny hole to put a pin thru so if the nut starts to come out,it will e stopped

if you could i would do a stud and bolt if possible, and have the stud welded into the manifold, this would solve the problem.

I'd think you'd need some kind of locking plate that would go between both bolts. Kind of like the locking plates on the stock turbo to manifold studs.

That'd be cool if the distance was the same or similar:
