Could be a pump going bad. Do u let it run dry between fill ups? Do u have a exhaust leak upstream from the 02 sensor?

i ran it low, never dry. no leak before the 02.

I ran this same setup in my last B14. This helped me alot.

Why not get a Calum B or RT or JWT ECU and call it a day. I had many problems with my SAFC2 and my stock ECU running rich as hell, but maybe you need some dyno time like i did.

what size injectors ?

i would recommend a calum or a jwt. stock ecu's in my opinion are just bandaids etc.

Are u sure your wideband is giving you good numbers?
An exhaust leak before the wideband makes your AFR reading lean.
And please calibrate that unit first.

Well i was going to buy a jwt ecu that a guy i knew had. But everyone said dont go with a jwt because they just go really rich. Just a off the shelf tune. Ill check out that link here and see if i can play with it.

what turbo? how many lbs of boost? i had the same problem on my t28 de+t, 370s, safc and stock ecu. couldnt get rid of that leanness for the life of me until i found out to my big suprise, i was maxing out the 370s on 10lbs with the t28. im pretty sure thats what your doing cause on the t25/t28 your rpm lean range is that sam rpm range boost kicks in the most! hope this helps. i got a jwt and they do run slightly rich but then again, you do have and safc to make up for that!

how much power were you making? were you running 4 bar fuel pressure?...
my 370s held up to 13.5 psi of T28 making 283whp on 92 se-r ecu + neo afc...
