Vadim, I've accidentally missed jury duty twice now... Once I'd forgotten and was 1,000 miles away. That was maybe 5 or 6 years ago. The other time I think I just straight-up forgot. Both times I called the hotline and left a message explaining what had happened and told them to reschedule me for another time. Nothing ever came of it. I've since been called and served three more times and it was seamless each time. I have no warrants out for my arrest. =]
