Oh, I never shut it down, left it on idle while making changes on the boost controller, that took me a couple of minutes, then I left. the spark plugs look ok, just a bit black from running rich.

I will check for sure the flow rates on those damn DW injectors. Maybe that cylinder was running too rich or too lean. The backfire came out of the tail pipe I could see the fire under the car Lol.

Technically that's not a backfire then (it's a forward fire?). Anyway... How does the cap and rotor look? Spark plug wires? Coil? etc. ?
Just chasing ghosts here.

Coil is an MSD blaster 2, stock spark plug wires in great condition, I haven't checked the cap and rotor. I will do it tonight.

before you remove the injectors make sure they are not leaking especially #4

Quick update, after disassembling the whole engine. Injectors flowed equally, spray pattern is good and none of them are leaking drops, so I can discard injectors as the reason of failure. Still pending to run the test on the AEM methanol system and see if its working the way it should.

There is some unusual wear on cylinder number 4, why would that be? why only on one cylinder? checking even further I noticed oil squirter for cyl #4 is broken wtf!!, could that be the reason, considering SR20DE don't even have them?

What is killing me is, I still don't know why I blew my head gasket at only 12psi

I think you should remove piston #4 to check for melted ring lands and such.
